Describe your educational background (degrees earned, post-graduate work completed, etc.).
 
List any professional affiliations or awards pertaining to unconventional natural gas or other petroleum resources or related interests.
Indicate specific areas of professional expertise as related to oil, gas, and/or unconventional natural gas or other petroleum resources exploration and production (i.e., reservoir, drilling, production, exploration, evaluation, environmental, safety, regulatory, etc.).
 
List your personal affiliation with industry/professional associations, societies, or other related groups.
 
Summarize your qualifications, if any, under any of the following membership requirements as listed under Section 999D(b)(2) of the Energy Policy Act of 2005:
(A): individuals "...who are employees or representatives of independent producers of natural gas and other petroleum, including small producers;"
 
(B): "...individuals with extensive research experience or operational knowledge of unconventional natural gas and other petroleum resource exploration and production;"
 
(C): "...individuals broadly representative of the affected interests in unconventional natural gas and other petroleum resource production, including interests in environmental protection and safe operations;"
 
(D): "...expertise in the various geographic areas of potential supply of unconventional onshore natural gas and other petroleum in the United States;"
 
Are you a contractor, subcontractor, consultant, or have any type of financial relationship with the Research Partnership to Secure Energy for America (RPSEA)? If so, please describe the nature of that relationship.
